Club Terms & Conditions

By registering to play with Pateley Bridge Junior Football Club, you (as parent guardian) agree that both you and your player will abide by the following Terms & Conditions of the Club.

  1. As a member of Pateley Bridge Juniors Football Club, the player agrees to abide by the Club Rules. The player agrees to observe the Club’s Anti-Discrimination & Equal Opportunities Policy and the Club’s Code of Conduct.
  2. I agree to the above-named child taking part in the activities of the club. My child will comply with the Club Rules. I accept that I have a responsibility for the actions of the above player.
  3. I agree to observe the club’s Anti-Discrimination & Equal Opportunities Policy and the club’s Codes of Conduct.
  4. I agree to collect my child from all training/matches at the appropriate time. I understand that players from the U6, U7 & U8 teams must have an adult present at all times.
  5. If my child is injured whilst training or playing football, I agree to allow my child to receive medical attention.
  6. If any of my child’s medical details change during the season, I will inform the club.
  7. I understand that if my child has asthma and doesn’t bring their inhaler with them they will not be allowed to play in a match or join the training session.
  8. I consent for the Club to hold the personal data given above according to the Club’s Privacy Notice and Data Protection Policy.
  9. I agree that the personal data given on the Club Registration Form can be used by the Club to enable the player to be registered on the FA’s Whole Game System and therefore take part in the Club’s activities.
  10. I consent for photographs/images of the above player to be taken and used by the Club for display, publicity and training purposes. (Optional)
